Is salicylic acid safe during pregnancy and breastfeeding?
Over-the-counter topical salicylic acid products (cleansers, toners, spot treatments) may be used during pregnancy if needed, according to MotherToBaby, because only a small proportion is absorbed through the skin. LactMed considers topical use acceptable while breastfeeding, as long as it is kept away from the nipple and skin your baby touches.
Also listed on labels as: salicylic acid, BHA, beta hydroxy acid.
Salicylic acid by stage
Trying to conceive
No concern found while trying to conceivePregnancy guidance allows OTC topical salicylic acid if needed, so there is no reason to stop while trying to conceive. Ask first about high-strength professional peels. [1]
Pregnant
No concern found during pregnancyMotherToBaby says OTC topical products containing salicylic acid may be used during pregnancy if needed. A relatively small proportion is absorbed through the skin, so it is unlikely to pose a risk to a developing baby. [1][2][3]
Breastfeeding
No concern found while breastfeedingTopical salicylic acid is unlikely to be appreciably absorbed or to appear in breast milk. Avoid applying it to the nipple area or skin your baby may touch or lick. [4][1]
Postpartum
No concern found postpartumThe pregnancy and breastfeeding guidance above already finds no concern with OTC topical use, and nothing changes after birth. [1]
What it is
Salicylic acid is a beta hydroxy acid (BHA) that unclogs pores. It is common in acne cleansers, toners, spot treatments and some dandruff shampoos, usually at 0.5–2%.
Why the stage matters less here
Only a small amount of salicylic acid applied to the skin reaches the bloodstream, which is why pregnancy and breastfeeding references treat everyday OTC products as low concern. Oral aspirin is a different question with its own guidance.
Practical tips
- Everyday cleansers and spot treatments are the products the guidance covers.
- Ask before chemical peels or large-area, high-strength treatments.
- While breastfeeding, keep it away from the nipple and from skin your baby touches.
